After an inquiry is made, Grane Hospice will handle everything. The admission process will begin immediately following a referral, no matter day or night. Our Altoona hospice staff handles all contact between family, facility and physician with real time updates.
If you wish to receive care in your own home, we will make every effort to make this possible. Grane Hospice Care will will obtain and review your clinical information, reach out to family members, and coordinate with the hospital or facility staff to make the transition back to home as easy as possible.
